Strange floating box displayed on survey page

On my survey page (http://www.surveygizmo.com/s/81721/wufo-customer-satisfaction-survey-nov-2008) there is a strange floating box that is displayed on page 1 of the survey.  It looks like a box displayed by the Firefox Cleeki extension (which I have installed).

So far, all the responses I have received were submitted using Firefox.  Several people using IE have reported that the Submit button is dead, so I suspect I somehow managed to stick some Cleeki code into the survey.  I can't for the life of me figure out where, or how to remove it.

Help!!

Hi, Chris.

Code has been added to your survey. We took a brief look at it and on question 4 if you go into Advanced Edit (the little notebook on the left side) and go to the Source code via the WYSIWYG editor, you will see all the extra code. Delete everything there except your question. Be sure and save changes. If you still see the box on your survey, that code is probably other places in your survey as well. You will need to open every question's Source code and look for the Cleeki code and remove it.
