*UPDATE*  Ok, the doofus found it – but it really shouldn't have been that difficult. Suggest making this an easier option.

I haven't found this elsewhere so I'm suggesting it – the ability to add a “Comments” section for each answer.

EG: Do you like kittens? YES | NO |-| Comment

The comment box should be visible whether yes or no is ticked.

Could this then be applied to a matrix of questions?

EG: Do you like kittens? YES | NO |-| Comment
     Do you like puppies? YES | NO |-| Comment
           Do you like fish? YES | NO |-| Comment

If this exists already Please let me know because I have seen no documentation on this.

Note: Survey Monkey (inferior product) allows for comments on question responses….

Thanks for your feedback! Please make sure the method you are using is not the following: http://www.surveygizmo.com/survey-blog/other-checkbox-radio-button/

Hi, thanks for the reply.

No, not using that method you linked to – after some hunting aorund I found that you need to have Pro account and then create a “custom matrix” question.

I hope that this option could be made simpler in the future (like a section to turn on/off comments in the advanced options area).

Thanks for a great product!

Just to give you an idea of what I mean – an easy way to add a comments box to any question (linking the comment in the results to the answer selected) would be fantastic.

Most of my surveys have Yes/No questions that the responder needs to be able to provide a Comment regarding their response.  As Dain mentions Survey Monkey provides this capability, that I got very used to before switching to Gizmo for other capabilities, so i would hope adding this function is somewhere on the near future development road map.

Dain, thanks for showing the option you used to get this done and saving me lots of time.

Hey, Rick (and Dain)!

This is actually very easy to do in SurveyGizmo.

We treat separate inputs as separate questions, but they don't have to look that way. I just made a simple sample survey with two questions, a rating scale and an open text field:

From there, I added in a “template hook” to the second question, and chose to skip question numbering for it.

And then went into my CSS to add a tiny line of code to target that template hook…

.floater .sg_QuestionTitle {
     float: left;
}

Ta-da! Look what we're got!
