Are there any plans to have custom radio buttons in the future? The standard ones look too plain. I have seen nicer ones (bigger, little check marks appear, etc.) on your competitors sites.

Yes, I have seen some of those. Quite cool.

Version 2 of SurveyGizmo will have some more options in that area. That release should happen sometime in 2009 but we have no firm date.
