I’m a grad school student wanting to conduct an embedded web-based survey. I need IRB approval in order to conduct this research (see previous posts what IRB is).

This is a response from IRB: “Most web-based surveys have the text for the consent as the first page and then this ‘yes’ ‘no’ box is connected. I believe that the researcher then receives a text version of their completion of the consent but perhaps not. In the consent letter there is a sentence that indicates they should print a copy for their records and that is typically sufficient for our review purpose along with their choice to proceed into the survey content.”

Do you have something like this? Or, is there a way to do something like this?

Hi Amanda,
Sure. Just create a page at the very top of your survey and add a single required checkbox.

The survey taker then has to check the box to get to the rest of the survey.

You can also have a copy of your consent agreement emailed to the survey taker. On the first page of the survey also ask them for their email address. On the second page you can add a Send Email action that will email the consent form to the email address the survey taker typed in.

So, just to be clear, surveygizmo.com does not keep track of such email addresses, correct? In other words, I want to be sure this is strictly confidential so I wouldn’t want any record of email addresses anywhere.

I’ve been playing with my free account and can’t seem to figure out the features you have discussed…adding a textbox and send email. Do I need to upgrade in order to do these things??

These addresses are only stored in your account, we don’t take a copy of email addresses for our uses at all. Your data is yours and is linked to your account and your reports/data, solely.

For your second question, yes, email notifications are listed under ADD ACTION (link at the top right of the page, blue box), and are available on the pay levels (personal accounts can only send emails to themselves, while pro and enterprise can send them to an email address provided in the survey).

Adding a textbox is simply adding a question and choosing the TEXTBOX type, which is the first one in the list.

Hope that clears it up a bit more!
