How do I add the IRB approval sheet before the survey?

Hi Lydia,
Uhh, sorry I don’t know what an IRB approval sheet is. Is it like a pdf/word document? If so you can just link to the file on your website.

An IRB (Institutional Review Board) sheet is a sheet that is an “approval” sheet that involves ensuring that various members of a “Board” approve a given research study. The research study usually involves
human subjects and the approval states that the research is appropriate for human subjects.


So the form is signed as a result of investigation by members (usually a variety of people with differnt baqckgrounds) of the Board whose signature assures that they feel that the research is ok to be performed on human subjects. Issues that the Board investigates include: appropriate informed consent (that the subjects understand important aspects of the reseach and what it is for), level of risk to the subject, confidentiality issues, whether human subjects are needed to test the “hypothesis(es), etc.


I would assume that Lydia is asking whether this form can be appended to the survey to show individuals taking the survey that the study/survey has recieved IRB approval. Another issue may be that individuals may need to electronically sign the form as part of the survey. And this would be interesting from my point of view relative to how the survey would capture an electronic signature.

Ahh, that makes sense. Well I think this is a two fold problem. The first is representing the document and the second is getting the signature.

So to represent the document you can either just include it in the survey as text in a question title, or provide a link to the document on your server (preferable one that opens in a new window).

To get the signature there are several methods depending on what level of identification you require. You can just include a single required checkbox, and all the person has to do is check the checkbox to proceed with the survey.

If you require a little more, you can also include a textbox that requires the person to type in their name.

If you require even more you can use our Survey Login feature. This will allow you to generate random one time use passwords that you can email to your survey takers and they will have to enter in to proceed with the survey. In this way you will know who used the password (by the email address you sent it to) and if they used them (they deactivate after use).
