Hello, Sanjeev!

I would per sonally recommend using our Ranking question type (with the table layout).

It won't look exactly the way it does in your example, but this is a question designed to work this way, and will give you the best output of your data once everything is over. here's how it would look:

In your example, can you force the response to 1 per column?
