I need to develop a survey that combines demographic questions and incorporates another questionnaire that has a copyright. I have the author's permission to use the questionnaire. The author granted permission to use the survey, provided that I keep the copyright information on the survey. How can I do this?

Hi, Linda.

Of course, you could use a copyright symbol somewhere in your survey to give the copyright credit.

Diid you have some other way in mind of giving credit to the questionnaire author?

Thank you, I will try the copyright sign. That makes sense. I guess I was trying to just import the entire survey, but what you have suggested sounds like it would work.
