2012/13 Nomination Form

Nomination Guidelines:

  • Both the nominee and the nominator must be current, full-time or part-time Finance & Administrative Services Division employees, including UARSC employees. 
  • You may not nominate the same individual multiple times.
  • Self-nominations are not accepted.  
  • Please exclude the nominee’s name in the descriptive portion of the nomination. Nominees’ names and any other descriptive information (department, projects) will be “scrubbed” and removed from the decision making process. 
  • All nominations are kept confidential unless nominator selects yes to disclose his or her name.
  • Either bullet format or essay format will be acceptable.
  • Nominations are not to exceed 200 words. Only the first 200 words of the nomination will be moved forward for the review and selection process.
  • In order to ensure an unbiased process, the FAS Awards Selection Team members (Steve Holbrook, Kathi Taylor-Elms, Hoami Thamracksa, Nick Tollison, Judy Sandmann, Sandra Young), the Vice President of FAS (Linda Hawk) and facilitator (Jennifer Williams) are not eligible to nominate, be nominated, or receive awards. 

STAFF may nominate up to three staff members within FAS for a Peer Recognition Award:

  • One staff member within your department
  • One staff member in another department
  • One  staff member either within or outside your department
  • Staff may submit a total of 3 nominations for 3 staff members (one staff employee within your department, one outside of your department, and one staff employee from any department within the division)

MANAGERS may nominate up to two managers within FAS for a Peer Recognition Award:

  • Managers may only nominate managers within FAS
  • Managers may only be nominated by managers within FAS

Responsibility: This individual takes ownership of actions, seeks out new opportunities and positive solutions, and is consistently reliable. This person can be counted on to successfully complete tasks and obligations.

Professionalism: Someone who applies their skills, knowledge, and experience to achieve outstanding results. A person who is consistently at his or her best, demonstrating good work ethics, standards, and techniques. The recipient of this award demonstrates the skill, competence and character expected of his or her profession.

Collaboration: This person exemplifies the word "teamwork." This is a person who is happy to pitch in, and strives to create a win-win situation. Cooperation could be his or her middle name!

Trust: Someone who is dependable and honest. This person can be counted on to follow through on commitments, keep information confidential when needed, acts credibly, and is generally thought of as an upright individual.

Respect: This individual shows respect to their colleagues and is respected in return. This person is someone who consistently treats other with respect and recognizes their colleagues' capabilities and contributions.

Service: This person is highly responsive to the requests of others. The individual enjoys giving his/her time and expertise. They make FAS their responsibility. They find the right balance of give and take while seeking and finding win/wins. This person chooses to assume their colleagues have good intent.

Laughter: This person finds humor every day. They have fun and share. They seem to be smiling about 50% of the time.

Inviting: This person greets people with a smile. Their workplace is professional and warm. They participate in friendly conversations that include words like yes, let's, and what if, which make them excited to come to work.

Excitement: This person brings passion to the work environment. They run into the storm because they know they can calm the storm. This individual models enthusiasm, energy, and competency. They are a magnet that attracts forward-thinking, helpful, and hopeful people who can be counted on. This individual creates a fun work environment.

Communication:This person creates a safe environment for listening, speaking, working, and living.

Innovation: This person finds and implements easier ways for helping teams and individuals get the job done. They come up with new solutions for getting better end results.

Finest: This person finds and implements the best practices from around the world that add to our positive reputation. They use their knowledge, authority, and sound judgment to contribute to FAS's reputation of excellence. Their experience, high credibility, and quality of service generate testimonials that praise the division.
