Informed Consent Form for Participants.

Thank you for choosing to take part in this survey. The aim of the research is to investigate how often we use our smartphones and what possible clinical implications overuse of smartphones may have in our society. To participate in the study you must own a smartphone and will be required to complete three components during the survey. The first component measures how often and for what reasons you use your smartphone. The second part is a personality questionnaire which measures authority, self-sufficiency, superiority, exhibitionism, exploitativeness, vanity and entitlement. The third and final section is a personality measure. The objective is to investigate whether or not there is a link between the subsections of personality and smartphone addiction. The experiment should take no longer than fifteen minutes to complete.

To ensure confidentiality, your data will be anonymous and only reported as group data; so there will be no identifying information. However you have until December 21st 2013 to withdraw your data from the study for any reason by emailing the principal investigator at 100146916@unimail.derby.ac.uk. In order to identify your data, but keep anonymity, you will need to enter a unique code below. This code must be made up of the first and second initials of your surname, the last three digits of your phone number and your year of birth. For example, John Smith, 07000222333, born in 1990 would be SM3331990.

Declaration of consent

I confirm that I have read and understood the information above and I have had the chance to as any questions I feel necessary before participation. I understand that my participation is voluntary and I have the right to withdraw, without giving reason, until December 21st 2013. I agree to participate in this study.
