Thank you so much for all your dedication and hard work in the Primary Prevention Initiatives. In order to be Leaner and more efficient in the submission of PPI proposals, we are now converting to an electronic format in Survey Gizmo to collect your PPI Proposal information. This Survey Gizmo link is replacing the Word Document Template that you have been completing and e-mailing to us. You will no longer need to email the PPI Word Document template to us because this will now serve as your PPI Proposal.
We will continue to pull information from this PPI Proposal survey gizmo link each week and email the spreadsheet to you to verify that the information we have is correct and up-to-date. When you receive the spreadsheet, please input the Completion Date (the date the PPI Objective has been completed) and email the spreadsheet back to Melissa.Blair@tn.gov
