PA Continuum of Care (CoC) Application Form

Please complete the following form to apply for membership to either the PA Eastern Continuum of Care Collaborative or PA Western Continuum of Care Collaborative, groups of organizations dedicated to serving those who experience homelessness or are at risk of experiencing homelessness.

The PA Eastern Continuum of Care Collaborative covers 33 counties in Central, Northern, and Northeastern Pennsylvania and is broken up into 5 Regional Homeless Advisory Boards (RHABs) to account for local differences within the broader context.

The PA Western Continuum of Care Collaborative covers 20 counties in Western Pennsylvania and is broken up into 2 Regional Homeless Advisory Boards (RHABs) to account for local differences within the broader context.

Anyone with a vested interest in homelessness and issues that relate to homelessness is welcome to join. Your selections in the choices below will connect you to the local RHABs, but will also add you as a general member of the broader Continuum of Care. This will enable you to be involved in the local level, while also receiving information that applies to all RHABs equally.
