In response to popular demand, the Department of Elementary and Secondary Education (ESE) Office of Special Education Planning and Policy (SEPP) is planning to revise the Individualized Education Program (IEP) Form.

We want your perspectives to drive the revision process. Your hands-on expertise is vital to the IEP Form Improvement Project in order to achieve an updated, meaningful and user-friendly form that works well for all educators, families and students. Help us answer these questions and design solutions to common issues.

With this initial survey, we will gather baseline data that will be used to guide the planning, development, and implementation of an improved IEP Form. Please understand that this brief survey is not designed to address the whole IEP process.  The information you provide will be used to determine the IEP Form Improvement Project priorities, serving as the foundation for future stakeholder input activities.

This 2-3 minute survey includes questions specific to individual sections of the IEP, as well as the overall form design.  Answering each question is not required, so you can skip entire questions if you prefer. Please provide as much information as you feel is necessary and be as candid as possible. This work would not be possible without your help.
