  • Your CURRENT FSF percentage
  • If you have set up an EFT with us or if you will send in your CHECK.
Use the tab key to go to the next field. If you do not have any sales in a particular field enter "0". If you have a EFT already set up with us then select Pay by EFT otherwise select Remit by check and submit your payment to the address provided. When this page is completed select NEXT at the bottom of the form.
