Please fill out the MLK Day of Service 2015 application to request funds for your service project on January 19, 2015. In this application, you will be asked to upload a W-9 form, letters of support and a budget. If you don’t have these documents ready, they can be mailed to State College of Florida. The mailing address is available at the end of the application.
