Purpose: In the ongoing effort to improve the facility survey process, we are asking for feedback regarding your individual facility experience with the surveyor(s). Feedback collected from evaluations will be used in the training and continuous education of surveyors across Tennessee.
Confidentiality: We ask for your honest and accurate thoughts and opinions. All responses are confidential and anonymous. No individual or facility identifying information (Name, Physical Address, Email Address, IP Addresses, Geo-Location or Invite Data) is captured unless otherwise explicitly provided by the respondent.
Estimated Time: The survey will take approximately 5-7 minutes to complete.
