Student Contract of Responsibilities for Transfer Students - Fall 2017

The Educational Opportunity Program (EOP) at Cal Poly, San Luis Obispo is committed to our partnership with you so that your educational experience will be a successful one. To achieve this goal, there are several commitments we will make together. Please read this contract, check “I accept” and submit online by May 1, 2017. We look forward to working with you!

I agree to fulfill the following: 

1.  Purpose of this document
This document outlines the commitments and expectations of the Educational Opportunity Program (EOP) and financial aid award.  While this is not a binding contract, failure to uphold these expectations may lead to additional interventions, or loss of program benefits.

2.  Meet with your EOP counselor at least once a quarter.
  • Your EOP counselor will be a key resource to support you from orientation to graduation. Plan to meet with them to obtain answers to a wide array of questions regarding:
    • Clarifying campus policies
    • Personal advising
    • Challenges at Cal Poly
    • Goal setting

3. Attend the SLO Days program (summer orientation program).

4. Complete these items during your first fall quarter, in order to keep your EOP grant eligibility.
  • First Year Seminar series during fall quarter 
    You will be expected to participate in an interactive seminar series sponsored by EOP that includes key topics: time management, university policies, career planning, study skills, university transition and more. Before fall quarter, you will receive information on how you can fulfill this part of your contract.
  • Supplemental Workshop in Math or Science 
    EOP expects you to include a Supplemental Workshop in Math or Science as part of your fall schedule. If unable to enroll, contact your EOP counselor.

5. Maintain full-time status (12 units minimum) each quarter that you are enrolled at Cal Poly.
  • If extenuating circumstances prevent you from enrolling or completing 12 units or more, connect with your EOP counselor.
