Online Performance Review

Introduction

Goal: The purpose of this Performance Appraisal is to strengthen the communication and better alignment towards goals between employee and supervisor.  Better communication, increased quality of work, productivity, and professional development are the target goals for the review process. 

Rating: The following ratings will be used to rate the employee's performance in the subsequent job functions. Please select the option that best describes the employee's performance.  
  • Outstanding- Employee consistently exceeds established goals, requiring little or no supervision, and with no errors.
  • Exceeds Expectations- Employee clearly exceeds established goals on a regular basis, with little supervision, and with minimal errors.
  • Meets Expectations- Employee meets established goals, requiring moderate supervision, and with limited errors
  • Improvement needed- Employee falls short of established goals, requiring significant supervision, and with significant errors. Requires constant follow up with supervisor.
Open Comments: In this section, you should provide concrete, measurable examples of the employee's performance in the specified area of review, for the review time frame.

Continue, Start, Stop: In this section, you should provide a limited number of specific tasks, behaviors, or skills that the employee should continue to, start to, or stop demonstrating pertaining to the corresponding category.
