This application for continuation grant funds includes end-of-year program reporting for current fiscal year 2014-2015 (FY15) grant activities, as well as proposed activities for FY16. Proposed activities for FY16 should be informed by lessons learned and needs identified by your participants as well as by ASOST-Q, Category A grantees through their continuation applications
(the Category A continuation applications are due August 4th – information provided about PD/TA needs will be shared by mid- to late-August).

Continuation grant funds must continue to be used to support OST programs, particularly those receiving ASOST-Q Category A (
quality enhancements) as they work to improve the quality of their programming.
