I hereby consent and agree that all family members waiver of all rights to any claims against Twellman Soccer Academy and further represent Twellman Soccer Academy from liability as follows. I represent by my registration hereto that all family members are in good physical condition and fully capable of exerting and performing without injury to themselves those physical functions required for participation. All family members agree to release Twellman Soccer Academy from liability for any and all injuries sustained as a result of participation. They knowing assume all risk for such injuries described herein and waive all rights to hold Twellman Soccer Academy liable for the same. All family members acknowledge that Twellman Soccer Academy assumes no liability for any personal items lost or stolen.
What is the cancellation policy after payment has been made? All cancellations are subject to a $30 processing fee providing that they are made prior to 14 days of the program start date. No refunds will be given for cancellations that are made within 14 days of the program start date. If you need to cancel or make changes to your program registration, please e-mail twellmansoccer@gmail.com.
Personal Trainings need to be canceled 24 hours prior or a $50 cancellation fee will apply.
In the event that a program is cut short due to inclement weather or any other unforeseen issues, every effort is made to make up lost time. In the event that the program cannot be made up, no refunds or credits are offered.
