PLEASE NOTE:

The participation fee for the 2016 Sousa International Honors Band European Tour is $3,995.00. Your application and earnest fee of $200.00 plus $300.00
should be mailed to Sousa International Honors Band, PO Box 102, Templeton, Massachusetts 01468-0102.
Further payment schedule outlined below.

I understand that the fee pays for my rooms, meals (demi-pension), and travel from the time I leave Logan International Airport, Boston until I return to Boston,
according to the conditions as outlined in the typical itinerary and information folder which accompanied this application and which I have read
carefully and which is part of this contract. See Tour conditions below. I have no health or physical conditions that would prevent me from
fully participating in the tour.

I will obtain a valid passport and I will be entirely responsible for my instrument and personal belongings, medical expenses, passport, and
personal and health insurance for the duration of the tour. Other than U.S. or Canadian passports may require visas.
TOUR CONDITIONS - Please read carefully

NOT INCLUDED - All items specifically mentioned in the tour itinerary or in the
conditions as stated above, included but not limited to passport fee; taxes, meals other
than stated above, mineral waters and other beverages not included on table d’hôte
menus; lunches except as stated in the itinerary; all items of a personal nature; personal
baggage and health insurance.

RESPONSIBILITY - the SOUSA INTERNATIONAL HONORS BAND acts only as
the agent for the participants on the tour, between the participant and the airlines and
the tour operator; and they shall not be responsible nor become liable for any delay
incurred by any person in connection with any means of transportation, nor for any loss,
damage or injury to persons or property by reason of any event beyond the control of
any agency or supplier in regard to travel, hotel accommodations, meals, and incidental
entertainment. The right is reserved to decline to accept or to retain at any time, any
person as participant in any tour. No refunds will be made for absence for any part or
portion of the tour. In the event of cancellation at any time after six days of receipt of
the application, the $200.00 deposit will not be refundable.

In the event of cancellation from February 15, 2016 to March 15, 2016, there will be a
$500.00 cancellation fee, includes $200.00 deposit. In the event of cancellation from
March 15, 2016 to April 15, 2016 there will be a $1000.00 cancellation fee, includes
$200 deposit. After April 15, 2016, due to the purchase of airline seats, motor coaches
and hotel payments, the refundable portion is limited. Cancellation and health insurance
is available for purchase. In case of cancellation of the tour by the SIHB ALL
monies will be refunded. Check your own health insurance to see if it is applicable in
Europe. All rates are based on current tariffs and currency exchange rates in effect
December 1, 2015 and are subject to adjustment, in the event of changes therein,
including possible increases in fuel cost or dollar devaluation. The tour price includes
all taxes and the fuel charge. However, if it is raised before departure, you will be
responsible for the added amount. The air carrier is not responsible for any act,
omission of event during the time the passengers are not on board their aircraft. The
passenger ticket in use by airline constitutes the sole contract between the airline and
the purchaser of these tickets, and/or the passengers. The right is reserved to make
necessary changes in the itinerary in the best interests of the tour. Coach travel is
insured.
