2016 SUCCESS Application

Instructions

Before you start filling out your application please prepare the following items ahead of time:

  1. Please prepare a list of previous research experiences, including the PI's name, Location, Dates, Project title and description. Please limit your response to 250 words per project. You will use this list to complete a portion of the application.
  2. Essay: Limit to 300-500 words. Please write a personal statement detailing why you would like to participate in The Ohio State University SUCCESS Program and how it is related to your career goals. Include any prior training or research experience relevant to the program and your goals. 
  3. Resume/CV
  4. For your 2-3 references please include contact information (name, email, phone number)
  5. Please visit the SUCCESS website (www.go.osu.edu/success) to identify 3-4 faculty members with whom you would like to work.
  6. Make sure you are eligible to apply: 
  • Expected College Graduation date in 2017 or 2018
  • Enrolled in a U.S. 4-year undergraduate institution
  • An American citizen or permanent resident
  • Please note that students at The Ohio State University that meet the above requirements are eligible to apply for the SUCCESS program
  • Individuals that fit the NIH Description of an underrepresented minority, has a disability, or comes from a disadvantaged background are especially encouraged to apply

Application Materials Checklist:

  • 2-3 Reference Letters (Your letter writter will be sent a link to submit the letter after your application is submitted)
  • Essay (via SUCCESS online application)
  • Resume or CV (via SUCCESS online application)
  • Transcript (send to the SUCCESS mailing address)
