I am a fourth year pharmacy student currently investigating customer attitudes towards online pharmacies as my final year project.
This questionnaire is a research tool used to gather information. The data you provide will be used for research purposes only. It will be kept strictly anonymous.
Access to the data is restricted, only the investigator and the project supervisor will have access to the information. All data will be handled securely and destroyed after 10 years.
We would appreciate the completion of all questions, however, you may stop the questionnaire at any time or leave questions unanswered.
