Carpentry Contest Entry Form

1. School contact information
2. We limit each school to 1 team of 2 contestants. We can take additional registrations but they will be reserves or alternates in case we do not have all available slots filled on the day of the contest. If one of your contestants are unable to attend, you can replace them with another of your students up to the start of the competition.

We do not need to know the team's makeup until the day of the event so you can determine this partnership at any point up until then. If you don't know the student's name, please enter TBD. Once you have a student selected please reach out to us to update your information..

Contestant information
In order to participate in the NC State Fair Registered Apprenticeship competition we will need to have you, the student, and their parent sign this release form and agreement to allow photos during the contest. You can either submit this agreement by fax to (919) 662-4557 Email to Apprenticeshipmail@nccommerce.com or bring it on the day of competition.

Without this page signed and on file on the day of the competition, your student will not be able to compete.

You will receive a copy of this registration by email after you have completed the entry, so you can print either this page, or the email for signatures.

VOLUNTEER AND PARTICIPANT WAIVER OF
LIABILITY AND PHOTOGRAPH RELEASE AGREEMENT

1. I, the undersigned, have voluntarily elected to participate in the Apprenticeship Contests sponsored by the N.C. Department of Commerce – Apprenticeship and Training (“NCWorks Apprenticeship”).

2. I understand and recognize the potential for injury to myself, to my personal property and to others and their property, which may result from participating in any manner in the Apprenticeship Contests. I understand, comprehend and appreciate the foreseeable, unforeseeable and inherent dangers and risks of harm involved in the Apprenticeship Contests, and I understand and comprehend that I agree to assume all such risks and dangers during my participation. Furthermore, I agree to use safe practices and tools when engaging in any activities associated with the Apprenticeship Contests.

3. I HEREBY RELEASE, DISCHARGE AND HOLD HARMLESS the N.C. Department of Commerce, NCWorks Apprenticeship, its principals, officers, agents and employees from and against any and all claims, liability and/or causes of actions for death, wrongful death, personal injury (whether physical, emotional and/or psychiatric or any combination thereof), loss of consortium, property damage and/or breach of contract made by or on behalf of the undersigned, the undersigned's spouse, children and heirs, occasioned by, arising out of or incidental to my participation in the Apprenticeship Contests, WHETHER OR NOT RESULTING FROM OR CAUSED BY NEGLIGENCE by, of and/or on the part of NCWorks Apprenticeship, its principles, officers, agents and employees.

4. I hereby grant to NCWorks Apprenticeship the absolute and irrevocable right and unrestricted permission concerning any photographs that they have taken or may take of me or in which I may be included with others, to use, reuse, publish, and republish the photographs in whole or in part, individually or in connection with other material, in any and all media now or hereafter known, including the internet, and for any purpose whatsoever, specifically including illustration, promotion, art, editorial, advertising, and trade, without restriction as to alteration; and to use my name in connection with any use if they so choose. I further understand that I am not entitled to any payment for such use.

5. I hereby release and discharge the N.C. Department of Commerce, NCWorks Apprenticeship, its principals, officers, agents and employees from and against any and all claims and demands that may arise out of or in connection with the use of the photographs, including without limitation any and all claims for libel or violation of any right of publicity or privacy.

6. I HAVE READ AND VOLUNTARILY SIGN THE VOLUNTEER AND PARTICIPANT WAIVER OF LIABILITY AND PHOTOGRAPH RELEASE AGREEMENT, and further agree that no oral representations, statements or inducements apart from the foregoing written agreement have been made.

Contest Schedule

7:30 a.m.–8:30 a.m. Registration/Tool Inspection and Setup
8:30 a.m.–9:00 a.m. Contestant Briefing
9:00 a.m.–1:30 p.m. Contest
1:30 p.m.–2:30 p.m. Judging
2:30 p.m. Awards Ceremonies

Requirements for Contestants

1. Must be a high school student in a related curriculum.
2. Must furnish own equipment and a complete set of tools including: hammer, level, framing square, combination square, tape, folding rule, hand saw, power saw (OSHA approved), 100' extension cord (OSHA approved), tin snips, cord/cordless drill, and #2 driver bit. Power saws with replaced cords and extension cords that have been repaired will not be approved.
3. Must wear proper footwear (no sneakers). Must have safety glasses, ear protection, leather gloves and hard hat available.
4. Must submit entry form no later than September 15, 2017.
5. Contest limited to 20 qualified teams of 2 contestants.
