2016 RIC WHF Military Sports Camp Application

Welcome to the RIC-WHF Military Sports Camp Application

Welcome to the 2016 RIC-WHF Military Sports Camp Application!

The Rehabilitation Institute of Chicago (RIC) and the Wounded Heroes Foundation (WHF) will host their 9th Annual Military Sports Camp in Chicago, IL on July 21 - 25, 2016. We welcome your application and appreciate your interest in participating.

The primary goal of RIC-WHF Military Sports Camp is to provide an introductory level sport experience for veterans and active duty military personnel with a primary physical disability. The timeframe in which you served, the location or cause of your injury will not preclude your application from being considered. The sports camp will feature thirteen different sport experiences including; Archery ● Biathlon ● Cycling ● Field ● Judo ● Kayaking ● Rock Climbing ● SCUBA ● Sled Hockey ● Swimming ● Wheelchair Basketball ● Golf ● and more! Each sport session will be led by an elite level facilitator so attendees have the opportunity to learn from highly skilled teachers.

Applying to RIC-WHF Military Sports Camp does not guarantee acceptance. Only first-time attendees of RIC-WHF Military Sports Camp are eligible to apply. The online application will remain open from February 15, 2016 - May 16, 2016. Completed applications will be reviewed by the application committee and the athlete will be notified of his or her acceptance status on or before May 20, 2016.

Flights, lodging, meals, local ground transportation, instruction, and program expenses will be provided at no cost to those accepted.

A great deal of support is provided to those attending camp. Last year almost 100 volunteers helped make camp a huge success. We encourage each individual to participate in every activity to the best of their own ability. If you are in need of medically-necessary support and would like to apply with a care giver, there is a section of the application to do so. Please understand, camp is made possible by the generous support of a number of individuals and organizations and we have a limited budget to support extra personnel. If you will need a medically-necessary care giver, you must list them on the application below. If accepted to camp, your care giver will be included in that acceptance. Those with a care giver will share a room with their care giver. Since your care giver is included in your acceptance, expenses such as flight, room, board, local transport, and other select activities will be provided free of charge to them as well. Due to limited staffing, care givers cannot be provided by RIC. Due to budget and hotel space constraints, we are unable to accommodate guests outside of those individuals accepted for camp.

This application will take roughly 15 minutes. You will be able to save your work at the end of a page and return to complete it at a later date. If you have completed a partial application and cannot get back into your work, please e-mail Derek directly.

If you have any questions related to RIC-WHF Military Sports Camp, please contact Derek Daniels at ddaniels@ric.org or 312-238-5008.
