COUN 5286: Supervised Fieldwork Experience in School Counseling (PPS) and Child Welfare and Attendance (CWA)

Guidelines

If you use the "Save & Continue" function at the top, SurveyGizmo will send a link to the email you provided. We tried this ourselves and it was sent to "Junk Mail". When we tried to access the link, an error box popped up saying, "To help protect your personal information, this message has been converted to plain text. Links and other functionality have been disabled. To restore functionality, move this message to the inbox." and prompts you to click "OK". You must "right click" on the message from SurveyGizmo and select: "Move" > "Other Folder" > "Inbox" > "OK". Once the link is moved to your inbox, you can now return to your saved Application in order to finish and submit.
Requirements for COUN 5286


ALL SUPPLEMENTAL MATERIAL MUST BE UPLOADED ONLINE ON TEVERA. 

If you do not have access to a scanner, you may use the Open Access Lab in King Hall D151 (in the basement) free of charge to prepare your documents so they can be uploaded to this application.

You may save and return to this form at any time by using the "save" feature at the top of the page. Please do not submit multiple applications. Instead, save and return to your initial application at a later time.

Before starting this application, you will need the following in PDF form to upload to Tevera:

1) A signed copy of your site supervisor contract (PPS/CWA) (Site Supervisor Contract)
2) Resume of your site supervisor (PPS/CWA)
3) Proof of meeting the Basic Skills Requirement (CBEST score report or Email approval from CCOE's OSS)
             Students can meet the Basic Skills Requirement through qualifying college-level coursework in lieu of the examination. To apply, please use the direct link below.

4) Certificate of Clearance  (COC)
You must provide your Certificate of Clearance  (COC) document number issued by the Commission on Teacher Credentialing.  To pull up your COC, go to the CTC website at: https://educator.ctc.ca.gov/esales_enu/start.swe?SWECmd=GotoView&SWEView=CTC+Search+View+Web and then click on secured search. You will have to enter your social security number and birth date. 

The online display includes the document number, document title, and issuance date.  Send in the document number for your COC.

5) Proof of CSULA's Student Liability Insurance

To purchase Student Liability Insurance- https://commerce.cashnet.com/csulapay
            Candidates enrolled in a fieldwork course are required to purchase liability insurance.  The liability insurance is good for one fiscal year (July 1 - June 30).   Students who are applying for Fall fieldwork, be sure to purchase your insurance after July 1 to ensure it will be in effect for the duration of your fieldwork.

PREREQUISITES
Students will be held responsible for the completion of all prerequisite coursework and other requirements. Students who have not completed all the requirements by the deadline will not be permitted to enroll in fieldwork. 

 
 Completion of the following courses:
  • EDSP 5000 - Foundations of Special Education
  • COUN 5000- Lifespan Human Development
  • *COUN  5002 (previously 5128)- Assessment in Counseling
  • COUN 5150- Ethical, Legal and Professional Issues in Counseling
  • COUN 5005- Individual Counseling Strategies
  • COUN 5230- Family Counseling Laboratory
  • COUN 5160- Group Counseling
  • *COUN 5281- Seminar: Leadership in Pupil Personnel Services
​*COUN 5128 and COUN 5281 can be taken concurrently with the fieldwork class in the Fall semester of the 2nd year.
 
 
AVAILABILITY FOR COUNSELING FIELDWORK: 
Students enrolled in COUN 5286 must be available to provide counseling services to their fieldwork site for the full 15 weeks of the semester. You are responsible for this requirement regardless of whether your school is off-track some part of the semester. You are responsible for making arrangements during off track time. These arrangements must be approved by the Program Coordinator or your Fieldwork University Supervisor prior to the beginning of fieldwork.

 
NOTE: 
All Application materials and documentation must be submitted online to the Division of Special Education and Counseling Office by the fieldwork deadline in the preceding Semester or you will be ineligible for fieldwork. All documentation is submitted online. No late applications will be accepted.
