Thank you for your participation in this year's survey. It should take approximately 15 minutes to complete. Your responses will be anonymous, unless you choose to offer your name at the end of the survey. Whether you choose to share your name or not, the results will only be reported on an aggregate basis.
We ask that each individual Partner in a household fill out the survey versus completing one survey per household.
The data gathered through this survey will fuel SV2’s ongoing learning and improvement, feed into SV2’s annual impact reporting, and support upcoming program planning efforts.
