RMAP Human Services Transportation Plan Survey

The Rockford Metropolitan Agency for Planning (RMAP) is updating its Human Services Transportation Plan (HSTP) to identify the strengths and weaknesses of the public transportation system in the region. To do this, RMAP needs information from users like you. The following survey should take no more than 15 minutes to complete. Information collected will be confidential and used solely for developing the RMAP HSTP.
