Dear Participant



Thank you for your interest in this survey. We are researchers based at St Mary's University Twickenham, and are conducting a study to explore issues around weight gain during pregnancy. There is a wealth of evidence that many women gain excess weight whilst pregnant and that this can have an impact on their long term health. Our study aims to identify some of the key reasons why women may gain excess weight during pregnancy.

We are interested in your latest pregnancy and would be extremely grateful if you could complete the questions below. All responses are anonymous and therefore will be strictly confidential and you will not be identified in any way. Please feel free to omit any questions that you do not feel comfortable answering and press the "Submit" button once you have completed the survey. We anticipate that this questionnaire will take 5 minutes to complete.

At the end of the survey you are invited to provide details separately to your responses should you wish to take part in an informal and friendly group session which forms the next phase of this study.
