The objective of this survey is to compile the experiences of PCAs and other Networks in evaluating and acquiring technologies designed to support health centers' needs around Population Health Management (PHM) and related data analytics. You have indicated that you have embarked on this process and that you would be willing to share the knowledge you've gained, which will be aggregated and disseminated to other networks looking to follow similar steps.
This survey will ask you a series of questions based on your experiences with a number of PHM analytics systems. We define these analytics systems as robust tools that assist in aggregating patient data across multiple technologies, often both clinical and financial, for the purpose of optimizing care management and patient outcomes.
