CONSENT TO PARTICIPATE IN RESEARCH

Barriers to Care and Primary Care Needs of Lakeview and Uptown

What is the purpose of this research?
We are asking you to be in a research study because we are trying to learn more about your primary health care needs as well as the barriers to affordable and accessible health care for Lakeview and Uptown residents. You are invited to participate in this study because you are an adult (at least 18 years of age), a resident of Lakeview or Uptown, and are uninsured, underinsured, on Medicare, or on Medicaid. This study is being conducted by Victoria Pressling, Stephanie Stuhlmuller, and Jennifer Walz, graduate students at DePaul University, as a requirement to obtain their Master of Science degrees. This research is being supervised by faculty advisors, Dr. Paula Kagan and Dr. Barbara Harris.

How much time will this take?
This study will take about 10 minutes of your time.

What will I be asked to do if I agree to participate in this study?

If you agree to be in this study, you will fill out a survey. The survey will include questions concerning your current health practices as well as basic demographic information. Some examples of demographic questions include age, ethnicity, education, income, and employment status.

What are the risks involved in participating in this study?

Being in this study does not involve any risks other than what you would encounter in daily life. For example, you may feel uncomfortable or embarrassed about answering certain questions. However, your name will not be attached to your survey and your answers will remain anonymous.

What are the benefits of my participation in this study?
You will not directly benefit personally from being in this study; however, we hope that what we learn will help the communities of Lakeview and Uptown better serve their residents by knowing the barriers that many people face in accessing affordable health care in their neighborhoods.

Can I decide not to participate? If so, are there other options?
Yes, you can choose not to participate. Even if you agree to be in the study now, you can change your mind later and leave the study. There will be no negative consequences if you decide not to participate or change your mind later.

How will the confidentiality of the research records be protected?
The records of this study will be kept confidential. All survey responses are anonymous. In any report that we might publish, we will not include any information that will identify you. Research records will be stored securely. Our faculty and the research office at DePaul University might review our records in order to make sure that we are doing what we are supposed to do. For example, the DePaul University Institutional Review Board may review the surveys. If they look at our records, they will not know who filled out the surveys.
