College Bound Scholar Survey: School Site Participation Interest Form

Welcome and thank you for your interest in our survey project to improve educational attainment for our College Bound students!  We will be collecting interest from schools in Washington through the end of January (NOTE: We have extended this timeline to collect additional interest).  At that time, we will select schools and notify both selected schools and all else who expressed interest.

By completing this form, you are not guaranteed participation. If your school is selected for participation, you will receive a $250 Amazon gift code for assisting us with this survey.

If your senior class's total enrollment is more than 200 students, you will receive a $500 gift card.

If you are the staff that will be coordinating survey administration at your school, please complete the short questionnaire below so that we may contact you. If another individual will be coordinating, please ask him/her to complete this questionnaire.

Participation Requirement:
If you are selected for participation, you will be asked to give paper surveys to all CBS scholars in their senior year at your school between February 1 2017 and March 31, 2017.  Importantly, you will need to give the surveys to ONLY your CBS scholars in their senior year.  The survey will take less than 10 minutes for each student to complete. You will then collect the surveys and send them back using a pre-paid envelope to the Washington Student Achievement Council. To receive the $250 Amazon gift code, you will need to achieve a minimum response rate of 75%.  That is, 75% of your College Bound seniors will need to complete the survey by March 31, 2017.
