We are doing research on a new approach to PrEP (pre-exposure prophylaxis) to prevent HIV. We have a few questions to see if you might be eligible. The survey will take a few minutes minutes to complete. Some of the questions ask about your sexual history. Your decision to take part in the survey is up to you. You may stop at any time and close your browser. We will keep your answers confidential and every effort will be made to protect your privacy. Your information will be stored on a secure serve and will be identified by a study ID number only. The information will be saved indefinitely to assess our studies’ progress.
If you are eligible based on these questions, you may be contacted to answer some more questions by telephone or to schedule a visit at our study site. We will ask for contact information from you so we can contact you. This information will be stored in a separate data file. Only the study staff will have access to these files. A risk of taking part in this questionnaire is the possibility of a loss of confidentiality or privacy but as we describe above, we plan to protect your privacy.
