NYC Service is excited to conduct its fourth annual Mayoral Service Recognition Program to document and celebrate corporate volunteer service across New York City in 2016. 
Please help NYC Service collect the best and most complete NYC volunteer data for 2016 by answering a brief survey.  The basic questions should take no longer than 20-30 minutes to answer.
To complete the survey, you will need the following information handy:
  1. The total number of employees in your company (all office locations).
  2. The total number of employees in your NYC office(s).
  3. The total number of NYC-based employees engaged in volunteering between January 1 – December 31, 2016.
* Please Note: Qualifications for the Mayoral Service Recognition Program are based on total number of NYC-based employees. See below:
  • 100 or more: 20%
  • Less than 100: 35%
