INVITATION
I would like to invite you to take part in a research study investigating nonsurgical cosmetic treatment and its association with various different factors. You MUST be 18+ to take part in this study and have a sufficient command of English.

WHAT WILL HAPPEN
In this study you will be asked to complete a series of questionnaires relating to:
  • Demographic & personal information (age, gender and ethnicity)
  • Nonsurgical cosmetic treatment
  • Social media use
  • Self-esteem
  • Body Dysmorphic Disorder
The study will take approximately 10 minutes to complete

CONSENT
By completing and submitting the questionnaire pack it will be assumed that you have given your consent to use the answers you have provided for the purposes of this study.

PARTICIPATION
Participating in this study is voluntary. If you agree to take part in the study, but at any point feel uneasy or uncomfortable, you are free to withdraw without explanation. You have the right to ask that any data you have supplied be withdrawn/destroyed. However after you have completed and submitted your questionnaire, we won't be able to withdraw you answers as they will be anonymous following submission.

CONFIDENTIALITY/ANONYMITY
The data collected in this study will not be linked to participants personally and will be anonymised using codes. All data will be kept locked away in a separate filing cabinet. Any data transferred to a computer/laptop will only be accessible via a password.
Following the completion of the study we will keep your anonymous answers to use for future research, including presentations and publications. 
This may include sharing these answers with others doing similar research but only if we approve their request to access it; it will not be given out without this approval. 
You will not be identified from this, your answers will remain anonymous.
