2017 Summer Migrant KEYS Academy Student Application

We would like to invite you to apply for the 2017 Summer Migrant KEYS Academy hosted by the Southern KY Regional Migrant Educational Office on Eastern Kentucky University’s campus.

Mission:
The Southern KY Regional Migrant Education Program K.E.Y.S. Summer Academy is a week-long program dedicated to helping students transition from middle school to high school by allowing students to focus on present and future educational/career goals, promoting self-awareness, and giving students the tools to help overcome barriers to success due to the migratory lifestyle.

Objectives:
 To encourage and provide opportunities for goal setting for the high school years.
 To build confidence through activities where sharing experiences and ideas are necessary.
 To encourage students to form social relationships and make new friends.
 To provide the opportunity to think about and plan for their futures after high school using the Common Core Standards of College & Career Readiness.
 To provide on-going interaction with role models who have completed a post-secondary degree.
 To encourage basic writing and to develop creative ways to express thoughts.

What is the cost of the 2017 Summer KEYS Academy?
 Our camp is FREE for eligible migrant students

Who can apply for the 2017 Summer Migrant KEYS Academy?
 Eligible migrant education students from the Southern Region in Kentucky.
 Incoming 7th – 12th graders.  PFS students and regionally served students will receive top priority.  We will accept between 40-50 students depending on funds.   

What do you need to bring for the 2017 Summer Migrant KEYS Academy?
 Comfortable shoes, tennis shoes, change of clothes for 1 week
 Pillow & Bedding Supplies 

The 2017 Summer Migrant KEYS Academy will supply the following:
 Toiletries, (Shampoo, soap, toothpaste, toothbrush, combs/brush, etc.)

Applications Due Date: Friday, May 19th
 The on-line application and ALL required permission forms must be completed and in the SKY regional migrant office by Friday, May 19th. No exceptions.
 The on-line application and PDF Waivers can be found on the EKU Migrant Education Website.

Application Process: Migrant Staff Only
Before completing and helping students/families with the on-line survey, you will need to complete the steps listed below.
1. Obtain the student’s ILP Username and Password

2. Print and obtain all required parent/guardian signatures.
 EKU Waiver
 Emergency Medical Waiver
 Photographic Consent & Release Form
 EKU Challenge Course Waiver
 Movie permission form

3. Ensure all forms are FULLY completed and have signatures
 You do not want your student to miss out on attending the KEYS Academy because you failed to review their application.
 Don’t assume all waivers are completed fully. Check them before sending to our office.

4. As mentioned, the on-line application and all PDF Waivers can be found on the EKU Migrant Website.

5. The on-line survey is simple and should only take about 10 minutes if you have the above information ready. The survey can be sent to parents via text and email however we recommend you assisting in the application process to ensure all paperwork and applications are complete.
