Invitation to Participate
You are invited to join a study. This study is led by Canyon Hardesty, Coordinator of Community Education at the Wyoming Wyoming Institute for Disabilities (WIND) and members of the Utah Regional Leadership Education in NeuroDevelopmental Disabilities (URLEND) program.

You must be at least 18 years old to join the study and one of the following:
  • A developmental disability (DD) provider (e.g. case manager, guardian, direct service provider, or other professional providing direct or indirect services to people with developmental disabilities) or
  • A medical provider or
  • A mental health care provider.
Purpose of the Study
WIND and URLEND members are asking for your participation in this survey so that the attitudes of DD providers, medical providers, and mental health providers regarding the sexuality of people with intellectual disabilities (ID) can be better understood within Montana, Utah, Idaho, and Wyoming. The study will assist members of the URLEND program to better understand provider attitudes regarding the expression of sexuality of persons who have ID. This information may provide future direction of work regarding how to best support the sexuality of people with ID.

If you agree to join the study, you will be directed to a web page in which you have the opportunity to provide demographic information. You will then be directed to a page that will provide statements regarding the sexuality of people with ID. You will be given several options indicating your level of agreement with each statement.

Possible Risks and Discomfort 
When you are answering questions, there is a chance the questionnaire statements could make you feel uncomfortable given that the topic of the questionnaire relates to sexuality. You may decide that you do not want to answer some questions. You may choose to skip questions in the Attitudes to Sexuality Survey or end the study at any time. If you choose to end participation in the study, you may exit the web page.

You are helping members of the URLEND program to understand current provider attitudes regarding the expressions of sexuality of persons who have disabilities, which can ultimately help inform future efforts to support the rights of people with ID. In addition, all participants will be entered into a raffle to win one of ten (10) $100 Amazon gift cards.

Information Shared Stays Confidential
Anything you answer on a survey will be kept confidential. All data will be de-identified and aggregated. All survey responses will be stored in a password protected database in WIND’s computer system with access only available to researchers involved in the project.

Freedom of Consent
Participation in this study is voluntary. You may end your participation at any time. Refusal to participate, or ending your participation, will not change your relationship with the University of Wyoming, WIND, or the URLEND program.
