PSO Culture Survey Registration Form

This is the registration form for the 2021 culture survey administration through the MHA Keystone Center PSO. Hospitals will have the option of administering either the Safety, Communication, Operational Reliability and Engagement (SCORE) survey, or the AHRQ Hospital Survey on Patient Safety (HSOPS). These surveys are administered free of charge to MHA Keystone Center PSO members. If your organization is not a PSO member and would still like to administer the culture survey through MHA, please email the Keystone Center PSO at keystonepso@mha.org.
