Letter of Introduction


Dear Participant -

My name is Natasha Smolkin, M.A., and I am a doctoral student in clinical psychology at the Wright Institute in Berkeley, CA. This survey is part of a study that I am currently conducting for my dissertation. The intention of the study is to take a closer look at the relationship between boredom proneness and relapse during substance use disorder recovery treatment. Your participation will contribute to data that will inform how we can better understand and treat substance addiction.

You may be eligible to participate in this study if you:

  • Have struggled with drug and/or alcohol addiction
  • Have tried to stop using drugs and/or alcohol
  • Are at least 18 years old
  • Live in the United States

If you choose to take part in the study, participation will involve the completion of a survey that will consist of questions about your experiences with substance addiction and recovery attempts. This survey will take approximately 15 minutes of your time.


In considering participation, please take into account the following:

  1. Participation involves minimal risk to you beyond the possibility of some mild anxiety brought in considering and responding to the topic and survey questions.
  2. Participation results in no direct benefits to you beyond what might be gained by the experience of participating in a research study, and contributing to a better understanding of the topic.
  3. This survey is entirely anonymous and your confidentiality will be protected to the full extent of the law. No identifying information is required to participate in the study or will be collected about you, or the computer or internet service provider through which you are accessing the website. No one, including the researcher, will be able to identify the responses of any individual participant or whether or not a participant chose not to participate.
  4. Your participation in the study is completely voluntary. You may discontinue participation at any time without consequences of any kind. Once you have completed the survey, you will be given instructions on how to how to receive a $5 Starbucks egift card.  You are not required to seek remuneration, although anyone who completes the survey is eligible.
