Background and Instructions for Completion

Background and Context
The Alliance for Better Health Care is seeking a Community Resource Assistance vendor to provide the following capabilities:
  • Identify programs, organizations and/or facilities to enhance community referrals options,
  • Develop real-time referral platform for referral content to support current Care Management infrastructure (Health Home, Care Management. Discharge Management, PCP office…) in identifying appropriate community referrals and supports,
  • Engaging members in self-use of system to support their needs requirements, and
  • Provide members connectivity to AFBHC resources to assist in care coordination.
Anticipated volume for the Community Resource Assistance center:
  • Initial (first 3 months): 25-50 requests for assistance/day
  • Established (3 months- 6 months): 75-100 requests for assistance/day
  • Maintenance (post 6 months): 100-150 requests for assistance/day
Along with incoming requests for assistance, the service centers will make periodic follow-up calls to monitor individual member experiences and identify additional care needs and problems that arise with daily. Member, Provider and Care Management satisfaction ratings are among the specific metrics used to evaluate the performance of a service center. AFBHC has a Salesforce CRM with partner's contacts and related information. And is in the process of deploying web based care management tool. The Community Resource Assistance representative must be able to use both these applications.

We serve an estimated 193,000 Medicaid and uninsured members of our communities across a six-county area in upstate New York (Albany, Fulton, Montgomery, Rensselaer, Schenectady, and Saratoga counties). The Alliance was formed in response to a New York State Department of Health initiative called the Delivery System Reform Incentive Payment Program (DSRIP).
