Certification Of Travel To Cuba - Under A General License For US Citizens and Permanent Residents

Effective January 16, 2015

Each person and/or organization traveling under one of the General Licenses categories for authorized travel must  retain specific records of the financial transactions related to this travel for 5 years. When traveling with ExperiencePlus! you are traveling on the People-to-People license as selected below per the activities that are organized by our local tour operator.  

Please fill this form out, sign it and print the PDF you will receive in the email confirmation. You will need to travel with this document as you may be asked to show your general license upon re-entering the Unite States. We recommend you also retain a record of the tour itinerary provided to you when you booked (also available to download online at www.experienceplus.com). 

I hereby certify that, at the time of travel to Cuba, I will be subject to the jurisdiction of the United States, including one or more of the following:
