Background
The Alliance for Better Health Care, LLC (Alliance) is seeking proposals from organizations to quickly deploy services and support to enable PPS success on Alliance DSRIP pay-for-performance metrics.

The focus of this RFP is for the last quarter of DSRIP measurement year 3 (MY3) performance period which begins on April 1, 2017 and ends on June 30, 2017.

The Alliance is open and actively seeking proposals with creative compensation/value-based payment methodologies and minimal up-front investment from The Alliance.

If your organization and its programs are successful in MY3, they will be considered for continuation into MY4 and MY5.

About The Alliance 
The Alliance for Better Health Care is a collaborative network of more than 2,000 care providers and community-based organizations who are partnering to transform how health care is delivered in our region, with a focus on Medicaid members and uninsured individuals.

We serve an estimated 193,000 Medicaid and uninsured members of our communities across a six-county area in upstate New York (Albany, Fulton, Montgomery, Rensselaer, Schenectady, and Saratoga counties).

The Alliance was formed in response to a New York State Department of Health initiative called the Delivery System Reform Incentive Payment (DSRIP) Program.

The shared goals of DSRIP and the Alliance are to achieve the so-called “Triple Aim” of improving patient care, improving the overall health and wellness of the communities we serve, and reducing health care costs. 
The Alliance for Better Health Care is seeking assistance in meeting a set of DSRIP performance goals for the current measurement year that ends in June, 2017. The successful partner will propose a set of activities that will assist The Alliance in meeting a subset of our performance goals that we may not be able to reach without such assistance.  We are interested in innovative solutions that directly address the needs of our community, while collaborating with (rather than replacing) existing community cased organizations primary care practices, accountable care organizations, and hospital systems



Sample performance goals and targets are below.  Our organization is focused on a subset of the DSRIP goals for this initiative.  The measures below reflect these goals, which should be reflected in any proposal.  We welcome proposals with scope beyond this subset.
