Instructions

This online survey is for reporting a blue-green algal bloom and/or human or animal illness that may be related to algal toxins.

The Wisconsin Department of Health Services, Division of Public Health (DPH), collects information about human and animal illness resulting from exposure to blue-green algae. Tracking illness information helps DPH measure the problem of blue-green algae in our lakes and rivers and learn more about the health effects of blue-green algae.

This program does not provide medical treatment, so if you are experiencing severe symptoms, seek medical attention right away or call the Wisconsin Poison Center at 1-800-222-1222.

If an animal is experiencing symptoms, call your veterinarian immediately.

If you are reporting food poisoning from eating fish or shellfish, please call 608-266-1120 instead of using this form.

In this survey, exposure means having contact with blue-green algae in water or algal scum, drinking or inhaling water with blue-green algae in it, or being very close to water with a blue-green algae bloom in it. An exposed person or exposed animal is the person or animal that came near or in direct contact with the blue-green algae.
