Congratulations! You're about to give your business a checkup. After answering the questions below, you'll receive a report that will allow you to assess the strengths and weaknesses of your business. As the CEO, owner, general manager or key executive, your satisfaction with the issues drives this report. The ratings and comments that you provide are confidential, so you can be comfortable being completely honest.

Here are a few things to keep in mind:

  • It's a self-diagnostic tool, and it is up to you whether or not you share the results with anyone.
  • If you're struggling with an answer, go with your "gut" or choose "neutral".
  • Please answer the questions independently. Several questions are about you, as the leader of your organization.

You'll need to complete all of the answers in one session. Don't worry, it shouldn't take you more than 10-15 minutes.

Our Return On Invested Capital is commensurate with our risk. (ROIC: A measure of how well you are using debt and equity to generate financial returns. Formula: Net Income-Dividends/Total Capital)
4. Debt to Equity *This question is required
Our debt/equity is at an optimal level.
5. Customer/Product Profitability *This question is required
We effectively measure which customers and products are most profitable, including overhead allocation.
6. Customer Satisfaction *This question is required
We have measurable, high levels of customer satisfaction.
7. Vision *This question is required
My employees would say that we have a compelling vision. (Vision: A clear, compelling picture of the future that aligns the organization and allows us to make decisions.)
8. Strategy *This question is required
My team can clearly articulate our strategy. (Strategy: An aligned set of actions that gives you a sustainable competitive advantage and moves you toward your vision.)
9. Reward System *This question is required
Our compensation/reward system drives the correct behavior.
10. Values *This question is required
My employees, customers and vendors would say that our actions are aligned with our company values.
11. Driving Force *This question is required
My team is clear on the driving force of our business. (Driving Force: The primary driver of your business, which you use to determine which products/service you'll offer, which customers you'll serve and what competencies you'll build.)

23. Exit Strategy *This question is required
I have an exit strategy. (Exit strategy: Most often, how a business owner plans to sell or transfer ownership of the business. In this case, we also include the executive who'll eventually leave his or her position to do something else, even if it is retirement.)
