As part of Fox Ridge Middle School's participation in the Colorado Department of Education (CDE) Bullying Prevention and Education Grant Program, we are asking for your permission for your son/daughter to complete a 6-item survey that asks if they have been bullied or seen other students being bullied at school. The purpose of the survey is to help us determine whether bullying prevention lessons (from the research-based Second Step curriculum) that we began using upon receiving the CDE grant have a positive effect on bullying in our school. The initial survey will be given in November of 2017, again at end of the school year, and twice in the 2018-19 school year. The questions we are asking have also been used on the CCSD Climate, Safety, and Wellness Survey that has been given district-wide every other year since 2000.
The surveys will be kept completely anonymous. No individually identifying information will be collected (all students will use the same link to access the survey) and there will be no place on the survey for students to put their names. In addition, each child will be given the option of leaving blank any question that he or she prefers not to answer.
Our school will receive a report with the survey results. All results will be presented only in group summary form; no individual names will be reported.
In accordance with district policy, we are asking permission for your son or daughter to participate in the student survey. Your child’s participation in the survey is completely voluntary. Should you choose to give your child permission to complete the survey, know that students can skip any question they do not want to answer.
