This application is for Wounded, Ill and Injured applicants for the 2018 Warriors to Lourdes Pilgrimage co-sponsored by the Archdiocese for Military Services, USA and the Knights of Columbus. This application should be completed by wounded, ill or injured individuals serving on active duty, honorably discharged, separated or retired from the United State Military (post 9/11 era). Individuals suffering from PTSD should also complete this application.
Please complete this application in its entirety and upload all attachments or send via email or mail no later than January 15, 2018. Incomplete applications cannot be considered.
Please note: Preference will be given to post 9/11 era warriors. Acceptance and full coverage for pre 9/11 era wounded, ill or injured veterans will be determined on a case-by-case basis. Also note, that designated caregivers and companions of wounded, ill or injured applicants must complete their own application.
