Alliance for Better Health Patient Engagement Support

Alliance for Better Health is committed to providing an infrastructure that empowers and engages individuals. The creation of this infrastructure is an essential component of our organization and its future.

In order to follow through on our commitment to create and maintain a community of health coaches, community educators and patient advocates, we are now seeking proposals from organizations interested in providing some or all of the services that are critical components to the creation of this infrastructure:

• A Patient Activation coaching curriculum to include core competencies such as:
  • Insurance Coverage
  • Health Care Team
  • Managing Cost
  • Use of medical services
  • Social Supports
  • Self-Management
• Mentoring of community-wide patient advocates and coaching team members, and
• A mechanism for performance assessment and evaluation of this workforce to ensure that the team achieves the following DSRIP goals:
  • Improved PAM Scores
  • Increased use of primary and preventive care services
  • Decreased preventable ED utilization

About Alliance:
Alliance for Better Health is a collaborative network of more than 2,000 providers and community-based organizations who are partnering to transform how health care is delivered in our region, with a focus on Medicaid members and uninsured individuals. We serve an estimated 193,000 Medicaid and uninsured members of our communities across a six-county area in upstate New York (Albany, Fulton, Montgomery, Rensselaer, Schenectady, and Saratoga counties). Alliance was formed in response to a New York State Department of Health initiative called the Delivery System Reform Incentive Payment (DSRIP) Program.
