Thank you for taking time today to fill out the Holly Hills 5th Grade School Climate Survey.
This survey asks about lots of different things that can have an effect on how well students do at school. Some of the questions are about you and some of the questions are about your school. They are all things that we cannot really know unless we ask you. They are also all things that we think are important to know about in order to make your school a positive place to be and to make it as easy as possible for students to learn.
This survey is ANONYMOUS. That means we
DO NOT want to know your name. We know that some of the questions are personal and we want you to know that we
cannot find out how you answer any of the questions – we hope that you will be honest and answer the questions based on what
you really do and think.
There are questions that ask about what grade you are in, if you are a boy or girl, and what your race/ethnicity is. We DON’T use these questions to find out who you are. We believe it is important to ask you for this information because it helps us know when some groups of students are feeling different about school than other groups of students –in the past we have found that these differences do happen in some classrooms.
All 5th grade students at Holly Hills will be asked to take this survey. The school will be given a report that lets them know what concerns 5th grade students have. The school will take this information seriously and try to use it to make positive changes.
If you don’t want to answer some of the questions, just leave them blank. How you answer the questions and how many questions you skip will not affect your grades in any of your classes.
