Washington All Payer Claims Database (WA-APCD): Registration for Review & Reconsideration


This survey asks for contact information about the person who will oversee the Review & Reconsideration (R&R) process for the Washington All-Payer Health Care Claims Database (WA-APCD) at your organization.

The State of Washington is creating a public website with information about health care organizations in Washington, including prices and quality. The site will open in 2018 and include information about your organization. A representative of your organization will be able to review the information before the site goes live and request corrections if needed. See the WA-APCD website for details about the R&R process.

On the following pages, please provide the name and other information for the person who will oversee this process, called the R&R administrator.

The R&R Administrator will be granted access to a secure internet portal where he or she can verify your organization's information and request corrections. The R&R administrator may delegate others from your organization to access the portal and help with the R&R process as needed.
