Health Quality Ontario (HQO) reserves the right to use any photo or video image recorded at the 2017 Ontario Surgical Quality Conference. By registering for this conference you hereby acknowledge and agree that HQO may photograph and/or video-record you at this event, as well as use the photographs and/or video recordings in any publication or media for future educational and promotional activities/materials, without further notification or any compensation to you. For example, selected materials will assist in the promotion of future HQO events and may be used in advertisements, print and online publications (such as reports), communications collateral (such as brochures and pamphlets), and/or on the HQO website. You also acknowledge and agree to waive any right to inspect or approve any future educational and promotional activities/materials that may include photographs and/or video recordings of you.
