I hereby give Lucile Packard Foundation for Children’s Health (the Foundation) permission to post in the Gallery of Gratitude at Lucile Packard Children's Hospital Stanford (the Hospital), and on the Foundation and Hospital's websites and social media the story and/or images that I have provided through the Gallery of Gratitude portal in substantially the same format as I have provided them. I understand that my story and/or the images provided will be posted on a public website. I understand that if any party mentioned in my story and/ or images should at any time request that the story and/or image be removed from the website, then the Foundation will immediately remove the story and/or image without further inquiry.
